| Phoenix Soccer Inc, Policy on the Collection, Use and Disclosure of Personal Information of Participants, Players, Coaches, Parents, Referees, Officers, Directors, Volunteers and All Other Participants Generally (the “Participants”).
Phoenix Soccer Inc, (Phoenix Soccer) is a non-profit volunteer based organization devoted to promoting, developing and administering soccer. As the PHOENIX SOCCER District Member for the north-east district of Winnipeg, Phoenix Soccer represents it’s participating Community Centre Soccer Programmes.
The vision of PHOENIX SOCCER is to be recognized as the Premier Soccer Club in the country and to have our program recognized as providing the most enjoyable soccer program in Manitoba and Canada. We will invest in the quality of our children’s experience by creating a soccer environment which enhances fun and improves participation, allowing children to play within age appropriate learning environments, with competent coaches, more game time, and lots of ball contact. We shall be the soccer club after which other clubs are modeled.
The mission of PHOENIX SOCCER is 'To provide a full range of quality soccer programs of all levels for Manitobans of all ages.'
Our philosophy is to promote fun, physical and intellectual achievement, sportsmanship, citizenship and recreation through the sport of soccer.
PHOENIX SOCCER shall value the game of soccer and strive to operate under a set of bylaws, rules and policies which ensures a fair, equitable, fun and successful soccer program at all levels. PHOENIX SOCCER shall value its volunteers and staff. The Club Executive and Directors shall value a team approach to the operation of the Club and value open communication to members and affiliates. Teams of individuals subject to senior executive approval under the current bylaws, rules and policies of the Club shall influence decisions.
Consistent with the vision, mission, philosophy and values, Phoenix Soccer is dedicated to maintaining high standards of confidentiality with respect to the information that has been provided to us. This Policy Statement has been prepared to affirm our commitment to maintaining the privacy of Participants and others and to inform you of our practices concerning the collection, use and disclosure of information provided to Phoenix Soccer Inc.
Our obligations apply to all officers, employees, contractors and agents who provide services to or on behalf of Phoenix Soccer in connection with our delivery of products, services and information to our Participants. Other applicable laws and internal Phoenix Soccer policies govern the protection of personal information of employees of Phoenix Soccer.
Phoenix Soccer collects information about Participants through its Player Registration Form, available in both a paper and, in future perhaps a Web-based format, and does so to fulfill the objectives and mandate of Phoenix Soccer and WYSA. The achievement of Phoenix Soccer’s objectives involves, among other things, communicating with and determining the needs of its Participants, providing services and information to its Participants, Fund-raising activities (such as web based corporate sponsors wishing to use direct marketing) to promote and operate soccer programmes, and permitting affiliated organizations and Preferred Suppliers to provide products, services and information to Participants. If a Participant consents, his/her personal information shall be used for these purposes. If a Participant does not consent, then his/her personal information shall be used by Phoenix Soccer for purposes of communicating with and providing services and information to the Participant only and shall not be provided to anyone else (i.e. Preferred Suppliers). In the event a participant does not complete the Collection, Use and Disclosure of Information Section of the Player Registration Form, the submission of a Participant’s Player Registration Form or update constitutes the consent of the Participant to the use of his or her personal information for these purposes.
If a Participant consents, the suppliers and sponsors of Phoenix Soccer and affiliated organizations who provide preferred services and products are to share with Phoenix Soccer and affiliated organizations information regarding the purchase of these services and products.
Phoenix Soccer also collects information about Participants through registration forms for a variety of programs including, but not limited to, player, coach and referee clinics, meetings and other professional or volunteer development activities. It does so to fulfill its objectives and mandate. In providing these programs, Phoenix Soccer is supported by Preferred Suppliers, Sponsors and Exhibitors, to whom a member’s name, address, email address and telephone number (“Participants information”) may be made available, provided the Participant consents during the registration process.
As a general rule, all Participant information is held in strict confidence and, except in limited circumstances, is not revealed to anyone unless expressly or implicitly authorized by the Participant. Phoenix Soccer does provide Participant information on a limited basis including Preferred Suppliers when:
· in the opinion of Phoenix Soccer, such usage constitutes a service and the information is pertinent to the Participants of Phoenix Soccer; and
· the Phoenix Soccer has the written agreement of Preferred Suppliers to adhere to specific requirements with respect to their use of such information.
A list of affiliated organizations, Sponsors and Preferred Suppliers is available from the Phoenix Office.
To illustrate these requirements, Phoenix Soccer Participant information:
· is not to be used in connection with any communication, which, in the opinion of Phoenix Soccer, would tend to mislead, misinform or deceive or which is distasteful in content or presentation or is illegal. A copy of the material(s) to be mailed is provided to PHOENIX SOCCER before the release of Phoenix Soccer Participant information.
· may only be used in connection with soccer legal publications or services deemed by the Phoenix Soccer to be pertinent to its Participants.
· may not be in connection with any fundraising efforts without Phoenix Soccer’s prior written consent.
There are circumstances where disclosure of personal information may be justified or permitted under a legal duty or right and Phoenix Soccer may disclose Participant information without consent. In such circumstances where, in the opinion of Phoenix Soccer, it is appropriate or necessary to disclose personal information, Phoenix Soccer will not disclose more information than is required.
Phoenix Soccer will retain personal information provided to it only for so long as it is required for the purposes stated above.
Phoenix Soccer endeavors to maintain the accuracy of any information about Participants in its possession. Information about a former Participant is not actively maintained and, for so long as it is held by Phoenix Soccer, Phoenix Soccer cannot assure the accuracy of such information.
While specific responsibilities may be assigned to other Participants of Phoenix Soccer staff, the General Manager of Phoenix Soccer is responsible for the administration of this Policy. In the event of any question concerning the access to or the accuracy, management or use of a Participant’s personal information, Participants should address such questions to the General Manager of Phoenix Soccer Inc.
Phoenix Soccer endeavors to maintain adequate physical, procedural and technical security with respect to our offices and information storage facilities so as to prevent any unauthorized access, disclosure, copying, use, or modification of a Participant’s personal information.
Visiting PHOENIX SOCCER Web Sites
A visitor to Phoenix Soccer Web sites is not required to reveal any individually identifiable information, such as name, address, or telephone number and such information is not collected passively by electronic means.
Information is collected when an individual voluntarily completes an online Participant application form or Player Registration form (not currently available but may be in future) or completes an online survey. This information is collected, used or disclosed in a manner consistent with this policy statement. E-mail addresses may also be collected during the online Participant application process but applicants may indicate that they do not wish to receive any unsolicited electronic communication.
Our Web server does not collect visitor information in the form of the visitor’s domain or Internet Protocol (IP) address but does collect information regarding which pages are accessed. This information is used internally, only in aggregate form, to better serve visitors by helping us to:
· manage our sites.
· diagnose any technical problems; and
· improve the content of our Web site.
Following its use, the information collected is not retained but, rather, is discarded in a secure manner.
Individuals who wish to do business transactions are requested to supply one or more items of data within the following classes of information:
· Corporate Identification Information.
· Personal Identification Information.
· Financial Information (eg. credit card information, purchase order numbers or any other payment information for billing purposes); and
· Delivery Information.
This information is collected for billing and delivery purposes as well as to permit us to contact you with respect to actions taken by us to complete your order.
If you purchase documents through these Web sites using a credit card, your credit card information is used only for credit card processing purposes.
For the transfer and receipt of certain types of sensitive information such as financial information, visitors will be re-directed to a secure server and will be notified through a pop-up screen on the site.
In browsing the Internet, you will encounter a technology called “cookies” which can be used to provide you with specific information from a Web site as well as provide the operator of a Web site with information about you. A “cookie” is an element of data that a Web site can send to your browser, which may then store it on your system. You can set your browser to notify you when you receive a cookie, giving you the chance to decide whether to accept it. This site does use cookies but only to record session information.
Phoenix Soccer does have a relationship with an ad server company but does not permit any ad server company to collect information about users such as domain type, IP address, and click stream information.
A user of these Web sites may access the information held by Phoenix Soccer about them in connection with their visits to this site as well as their account information. Users may request the correction of any inaccuracies in such information. Access to and, if required, correction of such information may be arranged by sending an e-mail message to the Webmaster of this site at info@phoenixsoccer.ca; telephoning or writing Phoenix Soccer at the contact information indicated below. Note that in some cases (for example change of address or domicile), Phoenix Soccer may request proof of the change under its rules and regulations before the change will be made or be considered binding upon Phoenix Soccer and other rules may dictate when such changes may be made, please check the rules applicable to the change, if any.
Evolving Practices
This Policy is in effect as of December 31, 2003. Phoenix Soccer will from time to time review and revise its privacy practices and this Policy. In the event of any amendment, a notice will be posted on Phoenix Soccer website and published in appropriate Phoenix Soccer publications. Policy changes will apply to the information collected from the date of posting to the web site as well as to existing information held by Phoenix Soccer.
Binding On Districts and Community Centres
Where a member Community Centre of PHOENIX SOCCER has not adopted its own specific privacy policy substantially similar to this one, it is the intention of Phoenix Soccer that such districts shall be governed by this policy and shall comply with it in accordance with the rules and procedures of Phoenix Soccer generally applicable to its districts and community centre operations.
