Referee Forms


Registration Forms

All individuals who are interested in becoming a referee must complete a Referee registration form and a waiver form.   Click here for Referee Registration Form.

For the Waiver Forms, if you are under 18 years of age, you must complete the U18 Waiver form or else please complete the 18+ Waiver form.

If you have any questions, please contact the Referee Convenor.

Reporting Forms

All Referees must fill out a report if there was a caution, dismissal or an special incident that occurred during the game you were refereeing.  Please complete the appropriate form or forms and submit it to the Referee Convenor immediately.
